Few vegetables are as bracingly bitter — and in the traditional systems of India, that bitterness is precisely the point. What the Ancient Texts Say

The Charaka Samhita places Karavellaka within the tikta skandha — the group of bitter herbs — and indicates it for prameha (urinary and metabolic disorders including diabetes) and medoroga (disorders of excess fat). Both the Sushruta Samhita and Vagbhata’s Ashtanga Hridayam list it within the Aragvadhadi gana, a classical group of purifying, metabolism-clearing herbs. The Bhavaprakasha Nighantu (16th century) describes Karavellaka as tikta (bitter) in taste and ushna (heating) in potency, acting as deepana and pachana (kindling digestion), rakta shodhaka (blood purifier), and above all as prameha-hara — a remover of diabetic and urinary-sugar conditions. Across these texts, one theme is consistent: the intense bitterness of Karela is understood as the source of its metabolic action.

What Modern Research Shows

Bitter gourd is among the most extensively studied plants in the antidiabetic literature, with over a hundred published investigations. Research has isolated three principal compounds behind its effect on blood sugar: charantin (a mixture of steroidal saponins), polypeptide-p (an insulin-like plant protein, also called p-insulin), and vicine. An early clinical report in the British Medical Journal (Leatherdale et al., 1981) documented improved glucose tolerance in people given Momordica charantia. A study in the Journal of Ethnopharmacology (Welihinda et al., 1986) found improved glucose tolerance in maturity-onset (type 2) diabetes. More recently, a 12-week randomized, placebo-controlled clinical study published in Food Science and Biotechnology (2022) reported that bitter melon extract lowered post-glucose blood sugar and suppressed glucagon in people with prediabetes.
